MADISON- Richard M. Carr, M.D., M.S., age 79, passed away on Thursday, July 3, 2014. He was born on January 27, 1935 in Great Falls, MT. Memorial donations may be made to either Our Lady Queen of Peace (401 S Owen Dr, Madison, WI 53711) or the Catholic Multicultural Center (1862 Beld St, Madison, WI 53713). Per his request, no formal services will be held for Richard.
