Philip A. Hoffman, M.D., passed away peacefully at Agrace Hospice on Wednesday, August 12, 2015. He was born on October 22, 1931 in Chicago, Illinois, the son of Philip and Ethel (Retzner) Hoffman of Shawano, Wisconsin. He graduated from the University of Wisconsin-Madison Medical School in 1957 and married Joyce Harrington Hoffman. He interned at Sacred heart Hospital in Eugene Oregon. During the same year, he and Joyce welcomed their first daughter, Susan. After touring duty with the U.S. Navy, he completed an anesthesia residency at the University of California in San Francisco, and returned to Madison, Wisconsin to join the Madison Anesthesiologists. He remained there for 38 years, until his retirement in 2000.
Phil was an accomplished wood worker, producing many beautiful pieces of furniture and museum-quality lathe-turned bowls. He enjoyed scuba diving, underwater photography, hunting out west with his cronies, and fishing trips in the Canadian boundary waters. The family cottage at Green Lake held a special place in his heart.
Phil is survived by Joyce, his wife of 58 years, and his five devoted children: Susan Hoffman, Cynthia Hoffman, Sarah Hoffman Carson, Mark (Jo) Hoffman, and Sandra Hoffman. He will be greatly missed by his six loving grandchildren: Daniel Carson, Hallie Mellendorf, Matthew Hoffman, Celia Mellendorf, Joseph Carson, and Ellie Hoffman.

A celebration in honor of Phil will held at Cress Funeral Home, 3610 Speedway Road, Madison, WI from 1:00 p.m. until 2:30 pm on Sunday, August 16, 2015, with a short service at 2:30 pm. Phil's family will receive guests before and after the service.
In lieu of flowers, memorials may be sent to Agrace Hospice Care, 5395 E. Cheryl Pkwy, Madison, WI 53711 or a charity of one's choice.
